Adult Single Point of Access & Accountability (ASPOAA)

The Adult Single Point of Access & Accountability (ASPOAA) is the process for referring adults to care management, Intensive Case Management (ICM), Assertive Community Treatment (ACT), and/or residential services. Oneida County offers a variety of residential options – ranging from independent supportive apartments to community residences with 24/7 staff supervision. The Adult SPOAA Coordinator will review each referral and determine the individual’s appropriate level of care. ASPOAA referrals are only valid if they are received and distributed by the ASPOAA coordinator.

To be eligible for ASPOAA services through Oneida County, applicants must be:

  • Over 18+ years of age
  • Willing to participate in these voluntary services.
  • Meet the diagnostic criteria for severe and persistent mental illness – as indicated in supporting documentation.
    • Supporting documentation of a psychiatric evaluation, or mental health assessment, indicating the individual meets criteria for a primary mental illness diagnosis, signed by a psychiatrist, medical doctor, nurse practitioner, LMSW, LCSW, LMHC, or LMFT dated within the past 12 months,
  • Marked difficulties in functioning as a result of their qualifying diagnosis.

NOTE:

  • Referrals that are missing required documents will remain pending until all documentation is received.
  • All referrals must be typed – the fillable form can be found below under Quick Links. Applicants will need to sign their consent on the referral. (Verbal consent is not acceptable)
  • All sections of the referral must be completed. Incomplete referrals will be delayed until all appropriate information is provided.

Non-Qualifying Diagnoses:

Primary diagnosis due to alcohol/substance use, dementias, organic brain syndromes, developmental disabilities, neurocognitive disorders, mental disorders due to a general medical condition or social disorders.
